Created by APS using applicant data report from Interfolio and sent to search committee for completion. Must be approved by APS & ODEI prior to proceeding with telephone interviews.

Submit completed forms directly to SBS 345 in the Accounts Payable drop-off bin.

Recommendation to Provost for final candidates for Academic Associate Dean recruitment. Must be approved by College Dean/University Library Dean and reviewed by the Senior AVP for Faculty Affairs and Human Resources and the Provost and Vice President for Academic Affairs prior to selection of final candidate.
