  • Recommendation to Provost for final candidates for Academic Associate Dean recruitment. Must be approved by College Dean/University Library Dean and reviewed by the Senior AVP for Faculty Affairs and Human Resources and the Provost and Vice President for Academic Affairs prior to selection of final candidate.
  • Recommendation to Provost for final candidates for Academic Dean recruitment. Must be reviewed by the Senior AVP for Faculty Affairs and Human Resources and the Provost and Vice President for Academic Affairs prior to selection of final candidate.
  • Request to offer appointment to final candidate for Academic Dean/Associate Dean recruitment. To be completed by the Office of the Provost and Vice President for Academic Affairs for approval of the President.
  • Previously known as the Form 5R: Reference Data for Recommended Candidate Reference information for candidate under final consideration for a position. Must be received by Human Resources and Academic Personnel Services prior to offer.
