eSignature Form Required for Open Enrollment

We are pleased to announce that HSU now has eBenefits.  This self-service platform will allow you to make open enrollment changes directly into your personal eBenefits account that is set-up for you in PeopleSoft.   However, before we can process any Open Enrollment changes in your personal eBenefits account, HR will need to have your eBenefits Self Service Electronic Signature Authorization Form on file.  

It is important that you review, sign, and return the original form and either mail it or bring in to Human Resources (Siemens Hall 212) as soon as possible, but no later than September 21stWe need a wet signature, and cannot accept this form if it is sent to us via email.
